Program Overview

The Santé Plus Access Membershi provides Care affordable, continuous, and culturally sensitive medical care to patients without active insurance. Members receive reduced-cost visits, preventive care, and lab discounts. This is not insurance, but a direct membership for primary and at preventive healthcare services Santé Plus locations.

Membership Benefits

4 in-person visits + 1 virtual visit per year.
50% off standard visit rate for extra visits.
Preventive screening(BP,glucose, cholesterol, BMI).
Discounted lab services ($60 and up).
Retina eye exam: $25.
Discounted diagnostics (EKG, ultrasound, X-ray).
Behavioral Health & Care Management Support.
Health education sessions (English, Spanish & Creole)

Membership Options

Additional fees will be applied for lab. Autopay is available for all payment options.

External referrals, emergency visits, and specialty services outside Santé Plus will be billed separately by the company that provides the service.

Payment & Renewal Terms

Payments accepted monthly, quarterly, semi-quarterly, or annually and can be processed by card, cash, or auto-pay.
Monthly: 3-month waiting period before starting.
Quarterly: Services start immediately.
Semi-Quarterly: 3 months upfront, then monthly. Services start immediately.
Annually: One-time payment with 5% discount; services start immediately.
Auto-pay available via card or bank transfer.
Membership renews automatically unless canceled 7 days before billing.
Missed payments beyond 30 days cause deactivation.

Administrative Terms

All documents and receipts are sent by email only.
Physical membership cards are pickup only at the clinic.
Autopay available for all plans.
Membership valid at Santé Plus Medical Center locations only.

Policies & Additional Services

Cancellation: 7-day noticerequired.
Non-Member Visits:

Standard $150 Initial + labs $225.

Additional Services: Labs, ultrasounds, physicals, procedures at discounted rates.
